The Care Group, P.C. provides integrative health care needs for ages 16 and older.

Note: We updated our billing policy for preventative care visits to comply with the Affordable Care Act. If you are scheduled for a preventative care visit and have a specific health concern that you would like to address, you can schedule a separate appointment. Or, if you would like to address your concern during your visit, please know that your insurance will code and bill the appointment as two separate charges.

Patient Pre-Registration

To make sure The Care Group, PC has current patient information, patients will be asked to fill out a brief demographic sheet at each visit. This will assist us in meeting your medical needs.

Please bring the following information:

  • Current ID, such as Drivers’ License

  • Current insurance information card

  • Personal Information (name, physical and mailing address, and phone numbers)

  • Parent/Guardian information (if necessary)

  • Reason for visit (i.e. annual physical, follow up medical visit, new ailment or symptom)

  • Employment information

  • Method of payment

First Visit

Please arrive 10-15 minutes prior to your designated check-in time and bring the following items to your first appointment:

  • Completed New Patient forms

  • Current medications

  • Current ID, such as Driver License

  • Current insurance information card

The front desk will confirm your paperwork is completed and signed where appropriate.

Appointments & Scheduling

To make an appointment, please call us in advance at 303-343-3121.

IN-OFFICE – It’s important to arrive a few minutes prior to your check in time if you’re an established patient to secure your scheduled appointment and to allow time for registration and paperwork at the front desk.

ONLINE – Virtual appointments (aka telehealth) are available and encouraged during the pandemic. A smartphone, tablet or laptop using a wi-fi connection will work with the platform used by your provider. Advance registration is also required and information shared when making the appointment.

Same-Day Appointments

Same-day appointments are available for patients who need to see a health care provider on an urgent basis. These appointments are filled on a first-come, first-serve basis so call early.

Late Patients

Patients are considered late if they don’t arrive or cannot be reached electronically by their check in time. While we will do all that we can to help late patients, we cannot always accommodate you and may have to reschedule.

Missed Appointments

To cancel an appointment, be sure to give a minimum of 24 hours’ notice. Failure to notify with less than 24 hours prior to your appointment is considered a missed appointment and is subject to a cancellation fee. of $100.00. Please see Cancellation Policy.

Cancellation Policy

The Care Group PC makes an effort to see patients on time in order to give patients the care they deserve. Therefore, we ask that you please give 24 hours’ notice if you are unable to keep your scheduled appointment.

We reserve the right to charge a cancellation fee of $100.00 in the event of missed appointments lacking proper notice. We will make exceptions in the event of reasonable emergencies.

Advance Payment Requirement

The Care Group PC requires a minimum $150.00 advance payment for all appointments for an individual new to our practice. The Care Group PC requires this payment because our providers and medical assistants reserve the appointment time specifically for you at the exclusion of other patients. The advance payment may be used as a credit on the patient account at patient’s discretion. The requirement is satisfied with payment via credit card when scheduling the appointment.

The payment requirement is subject to our Cancellation Policy.
