We are pleased that you have decided to visit our office. To make your experience more enjoyable, please follow these steps before you come to the office.  Don't forget to bring your insurance information as well.

Forms

There are three main forms to fill out for new patients. You will need Adobe Reader to view and print them. You can get the free Adobe software here. We will also be providing our forms in a "fill-in" version. These versions are editable and you can type in your information and print the completed form before coming into the office.

IBS Patients

If you will be seeing Dr. Guillory about IBS, please be sure to fill out this additional questionnaire before your visit. You can download it here or use the fill-in version which you can fill out and then print.

Please be sure to read our Privacy Policy. You will need to acknowledge that you received the Policy on the last page of the Health History form. You can find the Privacy Policy here.
