Information for new patients
Welcome to The Care Group, P.C. We are pleased that you have decided to visit our office. To make your experience more enjoyable, please follow these steps before you come to the office.
Insurance card
Please bring your insurance ID card so that we can bill your insurer correctly.
Forms
New patients will need to fill out three forms. If you wish to view and print these online, you will need Adobe Acrobat Reader. You can get the free Adobe software here. After printing the forms, you may fill them out at home and bring them to your appointment. We also are providing the forms in a "fill-in" version. These versions are editable, so you can type in your information and print the completed form before coming to the office.
These are the three required forms, in order:
- New patient profile - or - fill-in version
- Health history - or - fill-in version
- Optional information sheet - or - fill-in version
- Insurance and Financial Policy
- Patient Privacy Notice
- New Patients Policy Form
IBS Patients
If you will be seeing Dr. Guillory about IBS, please be sure to fill out the IBS Questionnaire before your visit.
You may download it or fill it out online. If you use the fill-in forms, you must print out the completed form and bring it to the office, as it will not automatically submit your information to us.

Last step
Please be sure to read our Privacy Policy. On the last page of the health history form, you will need to acknowledge that you received the policy. You can find the Privacy Policy here .
