Patient Portal also known as Health Tracker

At The Care Group, we know communication between doctors and patients is essential.

To that end, we are offering an online system that will allow you to review test results, request an appointment or request a refill of a prescription.

You can also update your personal information, access your health records. 

If you would like to register and get your personal pin number, please call The Care Group at 303-343-3121.

Important  - You must have an email on file with the Care Group,PC to set up your Patient Portal.

Log into www.healthtracker.com from the web.

1. If this is the first time you are logging in, you must click REGISTER on the lower right side to set up your account. See the yellow circle on the image below to to see where the Register button is.

2. You will then see the below screen shot.

3. -Choose a user name – and password.( Make sure you make a note of it for future use.)
   - Complete all security questions and personal information.
   - When you get to the Provider information - you are performing a search for your provider
     (so enter at least 2-3 characters of both the first and last names).
   - Fill in the City and the State and CLICK ON THE SEARCH BUTTON.
   - ONLY click on the search button AFTER you have put in the search criteria for your Doctor.
   - When you click search your Doctor (and all those that fit the search criteria) will come up
     (click on the correct one ONLY ONCE).
   - You will then see that your providers name has been selected.

4. Last step is to enter the registration number provided by The Care Group, PC. Then hit REGISTER.
